A Sensitive Enzyme That Points in Several Directions

Alkaline phosphatase is found in several tissues, and in dogs it is unusually easy to raise. That sensitivity makes it good at catching problems, but it also means a high ALP rarely points to a single answer on its own. Crucially, ALP is a cholestatic and inducible enzyme – it goes up when bile flow is impaired, and it is also “induced” (switched on) by steroids, certain drugs, the hormonal disease Cushing’s, and the bone activity of a growing dog. It is not a hepatocellular leakage enzyme like ALT. Understanding that distinction is the first step to reading a canine ALP sensibly rather than jumping straight to liver disease.

How GGT and the Panel Narrow It Down

The most useful companion to a high ALP is GGT, the other cholestatic enzyme. Because GGT is more specific to the bile ducts and is not induced by bone growth the way ALP is, the two together sharpen the picture: a high ALP with a high GGT makes a genuine bile-flow problem more convincing, while a high ALP with a normal GGT keeps drug effect, Cushing’s and bone growth higher on the list. An ALP that has sat above the interval for three years in a dog that remains well is a different situation from the same figure appearing for the first time this month, and only the previous pages can tell the two apart. What This Means for Your Dog Day to Day
For most dogs with a raised ALP and no other findings, day-to-day life does not change. There is no diet that lowers the enzyme, no supplement that has been shown to fix it, and no reason to restrict a dog that is well. What changes is that the value goes onto the monitoring list, and that any new sign is reported rather than filed away.
The observations worth making are simple. Water intake and urination, appetite, weight on the same scale, coat and skin condition, energy on walks, and the shape of the abdomen. Those are exactly the things that shift when the endocrine and hepatic causes of a high ALP develop, and they change before the next scheduled test is due.
Medication deserves a note of its own. Where a steroid or another drug is behind the rise, the answer is rarely to stop it – the condition it treats has not gone away – but to know that it explains the figure, so that the dog is not investigated for a disease it does not have. Never stop a prescribed medication because of a laboratory value; ask instead whether that value is expected on the treatment your dog is receiving.
Where a cause is found and treated, the enzyme is used to follow the response rather than to set it. It falls slowly, often more slowly than the dog improves, and treatment is not extended because a number lags behind an animal that is eating well, holding weight and behaving normally. That gap between the paperwork and the patient is normal and expected.
And where nothing is found, that is a legitimate result rather than a failure. What the Panel Around ALP Is Really Telling You
ALP is most informative in company. Read beside GGT, the two enzymes separate the biliary causes from the rest: both raised points at cholestasis, ALP alone in a young or medicated dog points away from it. Bilirubin adds urgency to that picture. A raised ALP with a normal bilirubin in a well dog describes a slow, tolerable process; the same ALP with jaundice describes something that needs attention now. Albumin, urea and glucose complete the assessment, because they report what the liver is still able to do rather than how much of it is leaking.
Outside the liver panel, the clues are just as useful. A dog with a high ALP, a dilute urine, a raised cholesterol and a thinning coat is describing an endocrine problem rather than a hepatic one. A growing dog with a high ALP and nothing else abnormal is describing its own skeleton. The value earns its meaning from what surrounds it.

Where Owners Most Often Go Wrong With This Result
The first mistake is comparing the value with a figure found online rather than with the interval printed on the report. Laboratories differ in method, and canine ALP intervals differ with them, so a result that looks frightening beside a remembered number may be ordinary beside the one that produced it. The report is the reference, not the search result.
The second is reading the height of the rise as the severity of the disease. In this enzyme, more than in most, a large number can have a benign explanation and a modest one can accompany something that matters. What separates them is the company the value keeps and what it does over time, not how far it sits above the line.
The third is acting alone – stopping a prescribed medication, starting a supplement, or changing the diet because of a laboratory value. The medication is usually treating something that has not gone away, no supplement has been shown to lower the enzyme, and a diet change made without a diagnosis simply adds a variable to the next panel.
The fourth, and the most common, is doing nothing at all because the dog looks well. A raised ALP in a dog that seems fine is exactly the situation where a planned recheck earns its keep, because the conditions worth finding early are the ones that look like nothing for a long time.
