When bile flow is impaired – a state called cholestasis – GGT rises, which is why an elevated level points to the bile system rather than to the liver cells themselves. The Lipidosis Clue: GGT Versus ALP

One feline pattern is worth knowing because it genuinely changes interpretation. In hepatic lipidosis – the fatty-liver condition that can follow a cat going off its food – ALP is typically high while GGT stays normal or only mildly raised. So the relationship between the two enzymes becomes a clue in its own right: an ALP that is up with a flat GGT fits the lipidosis picture, whereas a GGT that is clearly elevated alongside ALP points more toward bile-duct disease such as cholangitis. When Cholestasis Becomes Urgent

The GGT number itself is rarely the emergency – the cat’s condition is. Seek prompt veterinary care if a cat with a raised GGT also shows yellowing of the gums, eyes or skin, has stopped eating, is vomiting repeatedly, or is markedly lethargic, as these can signal significant cholestasis or a blocked bile duct. Appetite carries particular weight in cats: a cat that stops eating is at risk of further liver strain, so an off-colour cat with cholestatic markers should be seen rather than watched. What a Raised GGT Sets in Motion

Bilirubin is read next, because a duct that cannot drain backs pigment up into the blood; the liver enzymes are read alongside to show how much hepatocellular damage accompanies the biliary picture; and ultrasound looks at the gallbladder, the ducts and the pancreas sitting against them.
In cats the pancreas is part of this conversation more often than owners expect. The duct arrangement in the cat means pancreatic inflammation readily involves the biliary tree, so a cat with a raised GGT is frequently assessed for pancreatitis at the same visit. Inflammatory bowel disease completes the trio, and where all three are present the treatment plan has to address each of them rather than the most obvious one.
The contrast with hepatic lipidosis is worth holding onto, because it is the one pattern where GGT behaves differently from ALP. In a cat that has stopped eating and is mobilising fat into the liver, ALP typically climbs while GGT stays comparatively quiet. What follows depends on what imaging shows. A dilated duct with a stone or a mass leads toward intervention; a normal-looking tract with inflammatory markers leads toward medical treatment and a search for the underlying inflammation; and an equivocal picture in a cat that is eating and stable often leads to a recheck rather than to further procedures. In every version the GGT is the value that opened the question, not the one that answers it.
How the Sample Itself Can Change the Answer
Before a raised GGT is investigated, it is worth knowing what the sample can do to the figure. A haemolysed sample – one where red cells have broken during collection or transport – interferes with several chemistry results, and a lipaemic sample from a cat that ate shortly before the visit does the same. Neither means the cat is well or unwell; both mean the run may need repeating on a cleanly drawn, appropriately fasted sample before anything is concluded.
Method matters too. Laboratories differ in the analyser they use and in the reference interval they publish for it, so a value that looks alarming beside a figure found online may be ordinary beside the interval printed on your own report. This is the single commonest source of unnecessary worry for owners, and it is why your veterinarian reads the result against the page it came on rather than against a remembered number.
Timing is the third variable. Enzymes leak into the blood over hours and clear over days, so a sample taken at the height of an episode and one taken a week later describe different moments of the same disease. Where the clinical picture and the laboratory picture disagree, repeating the sample after a defined interval usually resolves the disagreement more cheaply than adding tests.
Finally, one normal result does not close a biliary question. Cats with intermittent obstruction or smouldering inflammation can look unremarkable between episodes, which is why a cat with jaundice, weight loss or recurrent nausea is investigated on the whole picture rather than discharged on a single acceptable value.
What Treatment and Monitoring Look Like
Treatment follows the cause rather than the enzyme. Where inflammation of the biliary tract is the driver, the plan addresses that inflammation and whatever is feeding it; where a duct is obstructed, relieving the obstruction takes priority and is a surgical rather than a medical decision; and where the pancreas or the intestine is involved as well, each part is treated rather than the most obvious one. Supportive care – fluids, pain relief, anti-nausea medication and getting the cat eating again – runs alongside all of them.
Nutrition carries more weight in the cat than in most species. A cat that stops eating begins mobilising fat into the liver, and that process can turn a manageable biliary problem into a second, serious liver disease within days. For that reason your veterinarian will push hard on appetite early, and may recommend assisted feeding sooner than an owner expects rather than waiting to see whether the cat comes round on its own.
Monitoring is a series of comparisons, not a single target. The enzyme is rechecked alongside bilirubin and the liver enzymes, on intervals set by how the cat is doing and by what treatment has changed, and it is read as a direction of travel. A value that is still raised but falling in a cat that is eating and gaining weight is a different situation from the same value in a cat that has stopped eating, and the plan follows the cat.

One last practical point. Keep the printed report rather than a summary of it, because the interval it carries belongs to the analyser that produced the value, and the next panel is only meaningful beside the last one.
