What the Letters ALT and SGPT Mean

Alanine aminotransferase is the modern name; serum glutamic-pyruvic transaminase, or SGPT, is the older label for exactly the same enzyme, and the two are interchangeable on a report. In the dog, this enzyme lives mostly inside liver cells, where it has a normal metabolic job and stays put as long as the cell membrane is intact. When liver cells are injured or stressed, those membranes leak and ALT escapes into the blood, where the chemistry panel measures it. The One Thing the Number Confirms – and Three It Cannot
A raised ALT points to hepatocellular injury: liver cells are leaking. That is the whole of what it proves. It does not say what injured them – inflammation, a drug, a toxin or disease elsewhere all look identical through this single value. It does not measure how much functioning liver remains, because a cell can leak ALT and still work. And it does not, on its own, tell you whether your dog is in trouble; a dog that is bright and eating with a mild rise is in a very different position from a dog that is flat and jaundiced with the same figure. A value just outside the interval in a thriving dog and the same value in a sick one are not the same finding, which is exactly why no single enzyme is read in a vacuum.
Why a Vet Reads ALT as a Liver Enzyme, Not a Muscle One

In the dog, ALT is treated as a liver leakage enzyme, and that is a deliberate clinical choice. Severe muscle injury can lift ALT, but only modestly, and only when the dedicated muscle markers CK and AST have risen alongside it. Without that accompanying pattern, a raised ALT is read as coming from the liver rather than the muscles. The Markers That Give ALT Its Meaning
A raised ALT becomes genuinely informative only once it is placed beside the values around it. Each neighbour answers a question ALT cannot – whether bile flow is blocked, whether the liver is still doing its job, and whether the dog is tipping toward jaundice.

What Tends to Push a Dog’s ALT Up

Because ALT only reports that liver cells are leaking, a wide range of situations can lift it. Rather than guessing from the enzyme alone, your veterinarian narrows the field using the rest of the panel, the dog’s age, the medication and exposure history, and a physical examination.

When the picture is murkier – a stubborn elevation in an older dog, say – the workup sometimes widens to ask whether a pattern across the panel points to something more systemic, which is decided on the whole report rather than on ALT in isolation.
When a High ALT Should Not Wait

The ALT number is rarely the emergency; the dog’s condition is. Seek prompt veterinary care if a dog with a raised ALT also shows yellowing of the gums, eyes or skin, has gone off its food, is vomiting repeatedly, or has become weak, wobbly or collapsed. A steeply climbing value paired with these signs is not a “wait and see” case, and the same is true when a high ALT sits next to a rising bilirubin. By contrast, a mild, isolated rise in a dog that is bright, eating and behaving normally is usually handled with a recheck rather than an emergency visit – the difference being the dog, not the figure on the page.
Finding the Cause, Then Watching the Trend
There is no treatment aimed at the ALT number itself; the work is to identify and address whatever is injuring the liver cells – a primary liver condition, a medication, a toxin, or disease elsewhere. What follows depends entirely on that cause: a veterinarian may stop or change a drug, provide liver support, or treat an underlying illness, and those are decisions made for the individual dog rather than from the enzyme value. Monitoring then means rechecking the liver panel – ALT alongside albumin, bilirubin and bile acids – at the intervals your veterinarian sets, and reading each result against how the dog is doing rather than chasing a target number. When the cause is treatable, ALT usually drifts back into range; when it stays up, that persistence is itself the cue to keep looking. The way two enzymes move together refines the picture further, whether ALT and ALP climb in step or one lags behind the other.
How a Raised ALT Is Worked Up in Practice
The first move is usually to confirm rather than to investigate. A single raised enzyme in a well dog is repeated on a fresh, fasted sample, because lipaemia, haemolysis and a difficult collection all interfere with chemistry results, and because biological variation alone explains a proportion of borderline figures. Confirming costs little and prevents an investigation aimed at an artefact.
If the rise is real, attention turns to the rest of the liver panel and to what the liver is still doing. ALP and GGT indicate whether the biliary system is involved; bilirubin indicates whether drainage is obstructed; albumin, urea, glucose and clotting times report function. A dog with a raised leakage enzyme and entirely normal function markers is in a very different position from one where both have moved.
Bile acids come next where function is in question, because they measure the liver’s ability to clear a substance it is responsible for, and they detect problems the enzymes miss – including the vascular anomalies that bypass the liver in young dogs.
Imaging then describes the organ: size, texture, masses, gallbladder and duct appearance, and the vessels around it. Where imaging narrows the field without naming the process, a sample of the tissue is the only way to distinguish inflammation from infiltration from fibrosis, and your veterinarian will weigh that against the anaesthetic and the dog’s stability.
What Owners Can Do While the Workup Runs
The most valuable contribution is an accurate account of exposure. Every medication and supplement in the house, anything the dog might have found on a walk or in a bin, any treatment given by someone else in the family, and any change of diet in the preceding weeks. Liver injury from something ingested is common and reversible, and it is usually the owner rather than the panel that identifies it.
The second is not to intervene independently. Supplements marketed for liver support are not a substitute for a diagnosis, some interact with prescribed medication, and starting one during a workup adds a variable that makes the next panel harder to interpret. Ask before starting anything, including over-the-counter products.
The third is monitoring at home in a way that produces a record: weight on the same scale, appetite, water intake, stool colour, energy, and any yellowing of the gums or eyes. These cost nothing and frequently change the urgency of the next appointment.
And the fourth is patience with the timescale. Liver values move over weeks rather than days, and a recheck scheduled for a month is not procrastination – it is the interval over which the organ can actually answer the question.
